Physical Therapy Assistant (PTA) – Home Health

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

Provide skilled physical therapy assistant services to patients in a home health setting across the East Portland area and surrounding communities
Visit up to 6 patients per day, maintaining a 1:1 care model with standard productivity expectations
Complete clinical documentation using EPIC Remote Client for Home Health and the Rover mobile application, including OASIS assessments
Manage daily scheduling and travel efficiently across an assigned patient territory, with occasional drive times of up to 60–90 minutes per day
Collaborate with the supervising physical therapist and care team to support patient treatment plans and progress

Required Qualifications

Active or pending Physical Therapy Assistant (PTA) license in the state of Oregon
Minimum of 1 year of skilled home health experience
Current Basic Cardiac Life Support (BCLS) certification
Valid driver's license and reliable personal transportation
Ability to work a 5x8 day shift schedule (no 4x10 or 3x12 availability)

Preferred Experience / Skills

Familiarity with EPIC, Rover, and/or OASIS documentation platforms
Experience managing an independent caseload in a home health or community-based setting
Strong time management and self-directed work skills suited for field-based care delivery
